Challenges & Approaches
The site was documented with laser scanning, photographic and video footage with proved invaluable during later analysis.
Extensive investigation and testing of mechanical systems identified the root cause of the incident.
Mechanical analysis and extensive field testing identified root cause of incident which was presented in the engineering investigation report.
Behavioural studies and systems review identified contributory factors for use by mine owner in preventing similar events in the future.
